Valid NYS Home Health Aide certificate Complete physical exam including PPD dated within the last 12 months (we will provide form for your MD) Must include chest x-ray positive dated within past year if PPD is positive Must include Rubella and Rubeola titers (attach lab results) Must include drug screen from past 12 months Two reference letters with telephone numbers Two pieces of current identification, one with photo Drivers License a plus When you need us, we are at your side Job Types:
